Welcome Phoenix Home Care employees!

Phoenix has arranged for staff to be able to order scrubs and accessories on the Scrubs On Site website under the following parameters:

- You can pay via credit or debit card or payroll deduction
- Shipping is free on orders over $50. It is $7.50 on under $50 purchases.
- Shipping will be to you through your Phoenix office.
- All tops and jackets are to be embroidered at a cost of $3 each. Purchases that fail to place the embroidery order, will be corrected and adjusted accordingly

To shop the Scrubs On Site web site:

1) Print these directions, then go to www.ScrubsOnSite.com
2) Click “Sign In” at the top right
3) Click “Create a New Account” at bottom left. Then enter Email and a password
4) You will receive an email to confirm address is valid. Follow the link to enter balance of information about you.
5) Your Account page will first appear.
a. Click the “3rd Party / Employer” link if payroll deduct purchase is desired
i. Enter “Group Billing Name” as provided by your Phoenix office
ii. Enter “Password” – as provided by your Phoenix office
iii. Enter your Employee Number
6) Once logged in, to access the Phoenix Home Care products and pricing:
a. click the "Group Sign In" at upper right
b. enter Phoenix-Home
c. the approved Phoenix Home Care products and pricing will appear
7) Place desired items into your “Cart.”
8) Once your Cart is ready, start the Checkout process.
9) Billing Information – pay via charge card, or choose the “Payroll Deduction” option.
10) Your orders should arrive approximately 2+ weeks after order.
